To submit a photo for the on-line Family Photo Album or Memorial Photo Album, please do the following: 

You may send your photo via e-mail or conventional mail. On the reverse side of the photo or in the body of your email, please write (a) the subject's name; and (b) your name, city, state, and phone number, as well as the country if you're outside the United States.

Email photos to photos@fsma.org.

We need your written consent to post your photograph on our website. Please print out the release form and complete and sign it.
  
After completing the form, mail it (and the photo if you are not emailing it) to the Families of SMA office:

If you would like a story to accompany your photo in the album, send it as text in an e-mail message to photos@fsma.org. Be sure to include the subject's name with your story so it can be properly matched with your photo.
